For children aged 14 years and younger, an annual Smart Smile Membership
Plan is just $435. For this low, annual fee, children receive all of the following benefits
- 2 dental exams each year
- 2 professional teeth cleanings each year
- 1 set of bitewing x-rays for cavity detecting
- 1 Panorex x-ray every 3 years
- 2 fluoride treatments each year
- 1 emergency exam and x-ray
- Complimentary CariVu images as needed
- 1 Sportsguard each year

Adult Plan – $466
For patients aged 14 and over, an annual Smart Smile Membership Plan is just
$466. For this low annual fee, patients receive all of the following benefits.

- 1 Panorex x-ray every 5 years

Periodontal Maintenance Plan – $835

- 4 periodontal cleanings each year
- 1 set of bitewing x-rays for cavity detecting
- 1 Panorex x-ray every 5 years

Terms and Conditions
The Smart Smile Membership Plan is not a dental insurance plan, and it cannot be used in any of the following situations:
- In conjunction with another discount plan or dental insurance policy
- In combination with services covered by workman’s compensation, medical, or general liability insurance plans
- To cover any treatment that, in Dr. White’s sole opinion, falls outside her capabilities
- For specialist referrals
- For hospital charges of any kind
- To purchase products
Payment Terms
Your annual fee for membership is due in full on your joining date. However, paying by credit card will have a service fee added to your membership price. In order to receive the 15% treatment discount, payment for service is due in full with cash or check. When paying with a credit card, debit card or Carecredit, patients will only receive a 10% discount. Your plan will renew annually on the original enrollment date. No refunds will be provided after dues are paid, including requested reimbursements due to failure to schedule and keep checkup appointments. Costs of annual dues are subject to change.
